Targets-Pharmacodynamics The primary pharmacologic response to isosorbide dinitrate is relaxation of vascular smooth muscle and vasodilation. These effects are due to the breakdown of isosorbide dinitrate at or near the plasma membrane of vascular smooth muscle cells resulting in the generation of nitric oxide. By activation of guanylyl cyclase, nitric oxide increases the production of cyclic GMP that in turn causes vasodilation, which results in decreased left and right ventricular chamber size and end-diastolic pressures without affecting systemic vascular resistance.
