They work by causing the kidneys to increase the amount of salts, such as potassium and sodium, that are filtered out of the blood and into the urine. When these salts are filtered out of the blood by the kidneys, water is drawn alongside them. As hydrochlorothiazide increases the removal of salts from the blood, it also causes more water to be drawn out of the blood and into the urine. Removing water from the blood vessels lowers blood pressure.

Kidney disease is common. It was the first angiotensin II antagonist to be marketed. As with all angiotensin II type 1 receptor AT1 antagonists, losartan is indicated for the treatment of hypertension. It may also delay progression of diabetic nephropathy and is associated with a positive clinical outcome in that regard.
A study hints that losartan has a beneficial effect on mitochondria by reversing age related dysfunction in maintaining normal blood pressure and cellular energy usage. Losartan is a selective, competitive angiotensin II receptor type 1 AT1 antagonist, reducing the end organ responses to angiotensin II. Losartan administration results in a decrease in total peripheral resistance afterload and cardiac venous return preload. All of the physiological effects of angiotensin II, including release of aldosterone, are antagonized in the presence of losartan.
Reduction in blood pressure occurs independently of the status of the renin-angiotensin system. As a result of losartan dosing, plasma renin activity increases due to removal of the angiotensin II feedback. Losartan is a uricosuric.

Changes in renal function including acute renal failure can be caused by drugs that inhibit the reninangiotensin system and by diuretics.
Despite these increases, antihypertensive activity and suppression of the plasma aldosterone concentration are maintained, indicating effective angiotensin II receptor blockade. After the discontinuation of losartan, PRA and angiotensin II values fell within 3 days to the baseline values.
Both losartan and its principal active metabolite have a far greater affinity for the AT1 receptor than for the AT2 receptor. The active metabolite is 10- to 40-times more active than losartan on a weight for weight basis. In a study specifically designed to assess the incidence of cough in patients treated with losartan as compared to patients treated with ACE inhibitors, the incidence of cough reported by patients receiving losartan or hydrochlorothiazide was similar and was significantly less than in patients treated with an ACE inhibitor.
