What is Order Combivir?
Side effects: Nausea, vomiting, fatigue, diarrhea, pruritus, anemia. In hemodialysis patients, every 7 days following completion of hemodialysis. Rarely, elevation of liver enzymes. Interactions, warnings: Patients with existing renal disease should either not receive tenofovir or, with no alternatives, reduce the dose.
Controls creatinine clearance and serum phosphate before starting therapy, during the first year of treatment every four weeks and thereafter every three months. Simultaneous determination of blood glucose and potassium, as well as glucose in the urine. Concurrent administration of tenofovir and drugs that are also eliminated via active tubular secretion can lead to increased serum concentrations of both drugs: cidofovir, acyclovir, valacyclovir, gancyclovir, valgancyclovir.
Side effects: The most frequent side effects are gastrointestinal, diarrhea and nausea.
The CDC currently considers the nucleoside reverse transcriptase inhibitors, including zidovudine, compatible for concomitant use with rifamycins, including rifampin, rifabutin and rifapentine. Rifapentine: Rifapentine appears to increase the glucuronidation of zidovudine, ZDV similar to other rifamycins. This may cause a decrease in zidovudine AUC.
However, the effectiveness of zidovudine against HIV does not appear to be altered. The activity of zidovudine is dependent on the intracellular concentration of the triphosphate metabolite which is not correlated with plasma concentrations of the parent compound. The CDC currently considers the nucleoside reverse transcriptase inhibitors NRTIsincluding zidovudine, compatible for concomitant use with rifamycins including rifampin, rifabutin and rifapentine.
Use caution and monitor for hematologic toxicity during concurrent use. The competition of lamivudine and trimethoprim for renal excretion, via active organic cationic secretion, results in delayed renal clearance of lamivudine. Use combination with caution and monitor for signs of lamivudine toxicity.
Teriflunomide: Zidovudine, ZDV should be used cautiously with other drugs that can cause bone marrow suppression including teriflunomide because of the increased risk of hematologic toxicity. In some cases, a reduction in the dosage or discontinuation of zidovudine may be warranted. Teriflunomide, an organic anion transporter OAT3 renal updake inhibitor, may cause elevated concentrations of zidovudine, an OAT3 substrate.
Tipranavir: Concurrent administration of tipranavir and ritonavir with zidovudine results in decreased zidovudine concentrations.
How should I take Order Combivir?
Additionally, because of the possibility of acquisition of another drug-resistant virus before treatment initiation, repeat resistance testing at the time therapy is initiated would be prudent.
Immune reconstitution syndrome has been reported in patients treated with combination antiretroviral therapy, including Combivir. During the initial phase of HIV treatment, patients whose immune system responds to antiretroviral therapy may develop an inflammatory response to indolent or residual opportunistic infections such as mycobacterium avium complex MACcytomegalovirus CMVPneumocystis carinii pneumonia PCPor tuberculosis TBwhich may necessitate further evaluation and treatment.
HIV treatment guidelines recommend all patients presenting with HIV infection undergo testing for hepatitis C, with continued annual screening advised for those persons considered high risk for acquiring hepatitis C. If hepatitis C and HIV coinfection is identified, consider treating both viral infections concurrently.
What should I avoid while taking Order Combivir?
Zidovudine: Zidovudine pharmacokinetics have been studied in a Phase 1 trial of 8 women during the last trimester of pregnancy. Zidovudine pharmacokinetics were similar to those of non-pregnant adults. Although data are limited, methadone maintenance therapy in 5 pregnant women did not appear to alter zidovudine pharmacokinetics.
The pharmacokinetics of lamivudine and zidovudine have not been studied in subjects over 65 years of age. There are no significant or clinically relevant gender differences in the pharmacokinetics of the individual components lamivudine or zidovudine based on the available information that was analyzed for each of the individual components.
Order Combivir side effects
There is increased myelotoxicity if used with other myelosuppressive drugs, especially gancyclovir but also co-trimoxazole, dapsone, pyrimethamine, interferon, sulfadiazine, amphotericin B, ribavirin and various other chemotherapeutic agents. Ribavirin antagonizes the antiviral activity of AZT in vitro combination should be avoided.
Initially monthly monitoring of blood count, transaminases, CPK and bilirubin. Gastrointestinal complaints can be treated symptomatically and usually subside after a few weeks. Generics to be expected.
