What is Adcirca?
Phenytoin Phenytoin induces the cytochrome P450 3A4 system. When both drugs are concomitantly administered, the clinical response to nifedipine should be monitored and, if necessary, an increase of the nifedipine dose considered.
Quinidine When nifedipine and quinidine have been administered simultaneously, lowered quinidine or, after discontinuation of nifedipine, a distinct increase in plasma concentrations of quinidine has been observed in individual cases. For this reason, when nifedipine is either additionally administered or discontinued, monitoring of the quinidine plasma concentration and, if necessary, adjustment of the quinidine dose is recommended. Therefore, the blood pressure should be carefully monitored if quinidine is added to an existing therapy with nifedipine.
Upon co-administration of both drugs, the blood pressure should be monitored and, if necessary, a reduction of the nifedipine dose considered. Cimetidine Due to its inhibition of cytochrome P450 3A4, cimetidine elevates the plasma concentrations of nifedipine and may potentiate the antihypertensive effect.
Rifampicin Rifampicin strongly induces the cytochrome P450 3A4 system.
Talk to your doctor to find out the true cause of any increased swelling you may have. Your doctor can help you decide how to manage it. For people with liver disease: Your liver clears this drug from your blood. For people with low blood pressure: If you have episodes of low blood pressure, taking nifedipine may make these episodes worse. This may be more likely to occur when you first start taking nifedipine, or when your doctor increases your dose.
Talk to your doctor if you have frequent episodes of low blood pressure. For people with a history of gastrointestinal blockage: There have been rare reports of gastrointestinal blockage with use of the brand-name drug Procardia XL. You may be at greater risk for this blockage if you have a history of it.
For people with severe heart disease: There have been rare reports of worsening chest pain and heart attack in people who have severe heart disease. Nifedipine capsules Procardia should not be taken within the first week or two after a heart attack.

What should I avoid while taking Adcirca?
In general, dose selection for an elderly patient should be cautious, usually starting at the low end of the dosing range, reflecting the greater frequency of decreased hepatic, renal, or cardiac function, and of concomitant disease or other drug therapy. Since this medicinal product contains lactose, patients with rare hereditary problems of galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption should not take this medicine.
Experience with nifedipine overdosage is limited. Symptoms associated with severe nifedipine overdosage include loss of consciousness, drop in blood pressure, heart rhythm disturbances, metabolic acidosis, hypoxia, cardiogenic shock with pulmonary edema. Additional liquid or volume must be administered with caution because of the risk of fluid overload. Clearance of nifedipine would be expected to be prolonged in patients with impaired liver function.
There has been one reported case of massive overdosage with tablets of another extended release formulation of nifedipine.
Adcirca side effects
Your dose, drug form, and how often you take the drug will depend on:The starting dose is 30 mg or 60 mg by mouth once per day.
The dosage can be increased every 7 to 14 days until the maximum dosage of 90-120 mg per day is reached. This medication has not been studied in children. The kidneys of older adults may not work as well as they used to.
